Here’s how they work:

- Download and complete the form with your vital medical information such as important phone numbers, preferred hospital, medical conditions, insurance information, and medications you take.
It even has boxes to check for Power of Attorney and “Do not resuscitate.” - Put the completed form in the vial and place it on the top shelf inside your refrigerator door.
- Put the Vial of L.I.F.E. sticker on the front door of your house or an adjacent window.
- Place the magnet on the front of your refrigerator.
Why the refrigerator? Because emergency responders know to look for the vial in that location if they see a Vial of L.I.F.E. sticker on or near your front door or the magnet on your refrigerator. What’s more, in case of fire, your refrigerator acts as a safe, protecting your vital information inside.
